WE WELCOME NEW PATIENTS

If you enrol at our practice your fees will be cheaper because we will receive a government subsidy for you.

In order to enrol we will require you to fill out an enrolment form. With your permission we will also organise getting your medical notes from your previous GP.

Every person over the age of 16 is required to have their own form.

You are eligible to enrol if you are a New Zealand Resident or hold a 2 year work visa in your passport. You will need your Passport or Birth Certificate as ID to enrol.

Please note, you can only be enrolled at one practice. If you enrol at another practice we will no longer receive a government subsidy for you and your fees will increase to that of a casual patient.

To enrol now please download our enrolment form, call into our practice, or phone and ask one of our receptionists to post you out a form.
 

HEALTH QUESTIONNAIRE

If you are a new patient please the health questionnaire with the enrolment form.

When you complete one of our enrolment forms, you are enrolling with Sunset Road Family Doctors. You do not have to choose a doctor, but if you have a preferred provider you can let us know.

If you are a new patient, please complete the electronic enrolment form below. It may take a few days for the practice to enrol you with the National enrolment service. Please note we cannot proceed with your enrolment without proof of eligibility (Passport or Birth certificate, Visa status)

We will email you to confirm we have received the registration form and will send through a health questionnaire to be completed as part of your enrolment.

If you have also consented to the transfer of your medical records from your previous practice, it may take up to 10 working days for these to come through, although you do not need to wait for this to book an appointment.
